Role of SureWell — Referral and Recommendation Only
SureWell helps you compare term-life options and hands you off to the carrier to apply. SureWell is not the issuer, administrator, or servicer of any policy. Your policy is issued by the named carrier; your contract, beneficiary designations, premium payment, and claims are governed by the carrier's documents and serviced by the carrier directly.
SureWell does not collect, hold, or transmit your insurance premiums. Premiums are paid directly to the carrier from a bank account you authorize during the carrier's application. You are responsible for ensuring the funding account remains active and sufficient to keep coverage in force.
Insurance Product Disclosure
Life insurance products available through SureWell are issued by licensed third-party insurance carriers and are subject to carrier approval, underwriting rules, policy terms, exclusions, limitations, and state availability. SureWell does not underwrite, issue, reinsure, or guarantee any insurance policy.
Carrier Identification
Term life insurance coverage offered through the SureWell platform is issued by one or more licensed, A-rated third-party insurance carriers, depending on product selection, eligibility, and state availability.
The issuing carrier, NAIC number, and state-licensed entity for your specific policy are identified in the application materials before you sign and on your policy declarations page.
No Guarantee of Coverage
Quotes shown during onboarding are estimates based on information you provide and carrier-provided pricing assumptions. Final premiums, coverage amounts, policy terms, and eligibility may change after underwriting review. Submitting an application does not guarantee approval or coverage.
Simplified-Issue Underwriting
Simplified-issue underwriting may rely on your application responses, identity information, prescription history, motor vehicle records, consumer reports, medical information bureau data, and other permitted sources. A medical exam may not be required for some products, but carriers may request additional information.
Policy Terms Control
In the event of any conflict between information displayed by SureWell and the policy documents issued by the carrier, the carrier’s policy, riders, notices, and endorsements control. You should review all policy documents carefully before accepting coverage.
Premiums and Lapse Risk
Coverage requires timely premium payment. If required premiums are not paid, coverage may lapse after any applicable grace period set by the carrier. Premiums are collected by the carrier directly from the bank account you authorize during the carrier's application. SureWell does not custody, transmit, or facilitate insurance premium payments. You remain responsible for maintaining coverage according to policy terms.
Producer and Compensation Disclosure
SureWell or its licensed insurance affiliates may receive compensation from insurance carriers when policies are issued or maintained. Compensation may vary by carrier, product, coverage amount, term, and state. This compensation does not increase the premium quoted to you unless disclosed in policy materials.
